Chronic Disease Management Plans
Chronic disease management (CDM) plans are available to people with a chronic or terminal medical condition. Your GP will work with the nurses and allied health to best support you and your health goals. With a chronic disease management plan, you may be able to access allied health services at a rebated price (podiatry, physiotherapy, dietician, diabetes educator etc).
A chronic medical condition is defined as one that has been or is likely to be present for 6 months or longer. Conditions may include:
*Asthma
*Cancer
*Heart Disease
*Diabetes
*Musculoskeletal Conditions
*Stroke
There is no list of eligible conditions; however, the Chronic Disease Management (CDM) Plan items are designed for patients who require a structured approach, including those requiring ongoing care from a multidisciplinary team.
A management plan can help people with chronic medical conditions by providing an organised approach to care. This plan helps to identify your health and care needs, and include working with a multidisciplinary team to achieve your health goals.
Once a plan is in place, it should be regularly reviewed by your GP. This is an important part of the planning cycle, where you and your GP check that your goals are being met and agree on any changes that might be needed.
